We are happy to provide paper copies of any of these forms for you.

Since 25th May 2018, the GDPR (General Data Protection Regulation) has been in force. We ask consent from you for different areas of school curriculum. We ask you to complete a consent form when your child starts at Rocks Park.
Parent Helpers
If you wish to become a parent helper you will need to complete a DBS (CRB) check. This form is available from the school office.

Please ensure that you have completed this form if someone other than a parent is collecting your child. Should your arrangements change, please contact the office.

If you wish to take your child out of school during term time, you will need to complete a 'Withdrawal from Learning form'.
This needs to be returned to the school office for the Headteacher to action.
Administering Medicines Form.pdfMedication
Medicines are not permitted in school unless your child has a chronic condition or requires prescribed medication. If your child needs medication during their time in school you will need to complete an 'Administering medicines form' and hand the medicine into the school office. Please ensure children do not carry any form of medicine on their person or in their school bags.
All medication (including asthma inhalers) must be in the original box.
